Performer's Insurance does not cover "athletic participation". It covers the performer should you get hurt during a performance or damage someone's property while performing (oops the hoop went right into the speaker! or you hit a spectator in the head with a hoop). There's no deductible and it's $210 per year. What I mean by 'separate waiver' is the insurance company issues an "Additional Insured Certificates" to cover the venue where you are performing. Some places require it. Others don't. If I know I'm performing in advance I just go ahead and request one and have them send it to me so I can keep it on file.
I always carry a camera with me when I perform because the policy says you need to document any incidents/damage etc.
For fitness classes you may need insurance to teach fitness classes. A waiver is a good 1st step but it is NOT insurance. You can still be sued and they will go after your personal assets. You can get fitness insurance even if you are not a certified fitness instructor (the premium payment is a little higher).
Ask the studio where you'll be teaching what type of insurance they need for you to teach. I don't know of a policy that covers both performer and fitness together.
